What is the difference between this and Remove pages?

They are opposites. Extract keeps what you name; Remove keeps what you do not. Use whichever is the shorter list to type.

Can I extract the same page twice?

Not here — a selection is a set, so a repeated page appears once. Organize PDF can repeat a page if that is what you need.
